The Order of Assessment and Driver Safety Plan that you received from the assessment agency required you to enroll in the Multiple Offender Program.

NOTICE: You have 1 year from the date of your assessment to fully complete a course. It is your responsibility to be aware of this date and to verify that the class you are enrolled in meets this requirement.

(If full payment has not been received by the payment due date, you will be removed from the class section and be reported to the Wisconsin Department of Transportation as noncompliant with your drivers safety plan).

ATTENDANCE:  Attendance in all classes is required. Failure to attend all classes will result in non-compliance that will be reported to the Wisconsin Department of Transportation. No refunds will be provided for failure to successfully attend all class dates as scheduled.

CONCERNED OTHER: You are required to bring a concerned other with you to class on November 9 & December 2. Failure to bring a concerned other with you to class on these dates will result in a failure of the course. No refunds will be provided for failure to successfully complete this course.

CONDUCT:  You must abstain from the use of alcohol and/or controlled substances (except prescribed medications) for 24 hours before class time. If you are under the influence of alcohol or controlled substances, you will be reported as non-compliant and will be required to contact the assessment agency for reassessment.

Course section # 63

REFUNDS and RESCHEDULING

Your registration reserves a seat in the class. No refund will be issued unless you contact the Traffic Safety Program to reschedule at least 72 hours before the class begins. Exceptions are only granted by the Traffic Safety Manager. When in doubt, call PRIOR to the first class date and ask.

If you miss the first scheduled class, you are responsible for registering and repaying for another class within 10 days. If you do not enroll in another class within that timeframe, you will be reported to the Wisconsin Department of Transportation (WI DOT) as non-compliant. You cannot miss the first class and continue in that section

Based on your Order of Assessment and Driver Safety Plan, you are required to complete the Multiple Offender Program.

PLEASE NOTE: You have one year from your assessment date to complete the course. Please review your assessment paperwork carefully and make sure the class you select will be completed before your deadline.

You must pay the full course fee by the payment due date. If payment is not received by the deadline, you will be removed from the class and reported to WisDOT as noncompliant with your Driver Safety Plan.

ATTENDANCE

Attendance at all class sessions is required. Missing a required class session may result in noncompliance being reported to the Wisconsin Department of Transportation. Refunds are not provided for unexcused absences or failure to successfully complete the course.

Excused absences are limited to illness, hospitalization, or a death in the immediate family. If you must miss class for one of these reasons, notify your instructor or the Traffic Safety Manager before class session begins. A written explanation and supporting documentation must be provided to the Traffic Safety Manager within 24 hours of the absence.
